UCPS Medical Consent 2018
- Each
student that requires any medication (daily or as needed) during the field
trip will need a UCPS medication consent form filled out by the physician
and signed by the physician. The school fax number is
704.243.0153.
- One
medication is required per UCPS medication consent form.
- If
the 8th grade student is also going on the DC trip in
March, the medication consent forms are still valid for the FL field
trip. However, the medication supply will need to be replenished to
the school nurse if needed.
- If
the student currently has medication/consent on file in the school health
room the medication will automatically go on the trip.
- Either
Mr. Mallery or Ms. Johnson will be administering medications to the
students in FL (the school RN will not be in attendance).
- Forms
and medications are to be turned into the school health room no later than
March 23rd. This is a hard deadline.
- Only
students with Type 1 diabetes or Asthma diagnoses are eligible for
self-carry.
- If
eye drops are prescription, they will need a medication consent. If
it is for the purpose of contacts, the student may self-carry.
- If
the student has a self-carry form present in the health room (such as an
inhaler), the student is expected to carry this at all times during the
field trip.
